Avoid paying an additional $600 in 2019 medical premiums

If you successfully complete the 2018 Wellbeing Assessment, you will not be subject to a $600 Wellbeing Assessment surcharge for your 2019 Allstate Medical Plan coverage (including HMOs). There are two steps to avoiding this surcharge:

Step 1: Get your biometric numbers

A biometric screening provides you with a scorecard of the leading indicators of your overall health — and the numbers you'll need for the 2018 Wellbeing Assessment. Biometric numbers from screenings completed at work will be uploaded to the wellbeing assessment for you. Learn more.

Step 2: Complete the online 2018 Wellbeing Assessment

The Wellbeing Assessment is a private, confidential and voluntary online questionnaire that helps you evaluate your health and wellbeing and provides suggestions for improvement. Anyone hired on or after July 30, 2018, does not need to take the 2018 Wellbeing Assessment.

You can still take the 2018 Wellbeing Assessment

Anyone taking the 2018 Wellbeing Assessment after 6 p.m. Central time on Sept. 21, 2018, will see the $600 Wellbeing Assessment surcharge reflected in their medical plan pricing during Annual Enrollment; however, the surcharge will not be assessed on your 2019 paychecks.

Two ways to access the 2018 Wellbeing Assessment through Virgin Pulse
  1. Virgin Pulse website, available via Your Benefits Resources™
  2. Virgin Pulse app. (Available on App Store and Google Play).

First-time visitors will need to register.

How to confirm you’ve successfully completed the Wellbeing Assessment:

Within 48 hours of successfully completing the assessment, Virgin Pulse will send you an email with the subject line, “Congratulations on completing the 2018 Wellbeing Assessment.” The email will be sent from “Donotreply@virginpulse.com.” Please keep this email until you receive your first paycheck in 2019 and have confirmed that you are not being assessed the $600 Wellbeing Assessment surcharge.

Who's eligible to take the 2018 Wellbeing Assessment?

All individuals employed by Allstate, Esurance or AFI are eligible to take the 2018 Wellbeing Assessment — as well as their spouses and domestic partners who are enrolled in an Allstate Medical Plan coverage option.

If my spouse/domestic partner doesn't take the assessment, will I be subject to the $600 annual Wellbeing Assessment surcharge?

No. No surcharge is assessed if your spouse/domestic partner does not complete the assessment.

Spouses/domestic partners who want to take the Wellbeing Assessment may take it using their personal Virgin Pulse account at join.virginpulse.com/allstategoodlife (first-time users will need to register).

What if I don't take the 2018 Wellbeing Assessment?

Although we encourage all those who are eligible to take the 2018 Wellbeing Assessment, doing so is voluntary. If you do not take the by December 31, 2018, at 6:00 p.m. Central time and you:

  • Enroll in the Allstate Medical Plan for 2019, (including HMOs), you will be assessed a $600 Wellbeing Assessment surcharge for your 2019 medical coverage.*
  • Do not enroll in the Allstate Medical Plan for 2019, you will not be assessed the surcharge.

*Individuals hired on or after July 30, 2018 who do not take the 2018 assessment are not subject to the Wellbeing Assessment surcharge in 2019.

How long does the 2018 Wellbeing Assessment take to complete?

Approximately 15 minutes.

Why does Allstate have a Wellbeing Assessment?

The information from the Wellbeing Assessment can be an important preventive tool for you. Because some health conditions may have no symptoms, you could be at risk without even knowing it. With the information you learn from the Wellbeing Assessment, you can be better prepared to identify and address health risks early on that could possibly affect the quality of your life – and your health.

In addition, the aggregate data we receive from the Wellbeing Assessment provides us with a picture of the general wellbeing status of our population. This information is valuable because we use it to identify and prioritize:

  • Corporate wellbeing programs that would best support the needs of our population.
  • Ways to design our Medical Plan to help improve the overall health of Plan participants and control the overall costs of medical coverage for both Plan participants and the Company.

For example, based on Wellbeing Assessment data collected in previous years, we have recently offered programs for stress, nutrition, sleep and financial wellbeing to support employees/associates in improving these aspects of wellbeing.

Why do I need biometric numbers to complete the Wellbeing Assessment?

Providing biometrics numbers helps you get a more accurate idea of the current state of your health and helps us get a more complete picture of the current wellbeing of our population.

How recent does my biometric data have to be?

Your biometric screening results for the 2018 Wellbeing Assessment must be obtained any time after Sept. 1, 2017, in order to be used on the assessment.

Keep in mind, if you participated in a screening at the office this year by August 20, 2018, your biometric numbers will be automatically uploaded to the Virgin Pulse website within two weeks of your Assessment. They will also be uploaded to the Wellbeing Assessment for you. Once the numbers are uploaded, you will not be able to change them.

If you wish to see the biometric numbers that have been uploaded for you, there are two ways to do so:

What if I had a biometric screening in 2018 through an onsite screening or as part of my preventive exam at the Allstate Good Life Wellness Center?

If you had your screening at the office by August 20, 2018, your biometric numbers will be uploaded to your Virgin Pulse account within two weeks of your screening. Once the numbers are uploaded, you will not be able to change them.

If you wish to see the biometric numbers that have been uploaded for you, there are two ways to do so:

   OR

How confidential is the Wellbeing Assessment?

We take the requirements of the Health Information Portability and Accountability Act (HIPAA) very seriously and are compliant with the appropriate provisions of the Act.

The assessment is administered by Virgin Pulse, our vendor for wellbeing programs. Virgin Pulse is a third-party that is completely separate from Allstate, Esurance and AFI. Virgin Pulse only provides results to Allstate in aggregate form without any individually identifiable information, allowing us to see our population's overall health and wellbeing trends — and how our results compare with national averages.

No one at Allstate, Esurance or AFI (including your manager, co-workers, or officers) sees or has access to your individual responses to the Wellbeing Assessment. Responses cannot and will not be used for any employment or performance-related decisions.

Who sees the data from the Wellbeing Assessment?

Virgin Pulse collects and stores onsite biometric data and responses from the Wellbeing Assessment. The company only receives data in aggregate form without any individually identifiable information. The information from your Assessment and biometric screening data is also provided to Truven, our data warehouse vendor, and may be used to provide aggregate data reporting and analytics.

Why are we asked to take the Wellbeing Assessment every year?

By conducting the Wellbeing Assessment on a yearly basis, we are able to monitor our population's wellbeing needs as well as the success of our wellbeing efforts. This allows us to adjust our corporate wellbeing programs and our medical plan design from year to year, as needed. It also provides you with a current picture of the status of your health and wellbeing.

How secure is my information at Virgin Pulse?

We take the security of your information very seriously. Allstate vendors, like Virgin Pulse, who receive certain types of confidential information are required to meet certain detailed security standards imposed by Allstate.

The Allstate Good Life Wellness Program is a voluntary wellness program available to all employees. The program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. As part of the wellness program you will be invited to complete a voluntary wellbeing assessment or "WBA" administered by Virgin Pulse. The WBA asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You will also be asked to complete a biometric screening, which will include a blood test for cholesterol, glucose and triglycerides, as well as information about your height, weight, waist-circumference, and blood pressure. You are not required to complete the WBA or to participate in the blood test or other medical examinations.

However, employees who choose to participate in the wellness program will avoid an annual Wellbeing Assessment surcharge of $600 for completing the WBA. Although you are not required to complete the WBA or participate in the biometric screening, only employees who do so will avoid the surcharge.

Also, other incentives may be available in 2019 for employees who participate in certain health-related activities or achieve certain health outcomes. Additional details will be provided. Additional surcharges of up to $600 may also be imposed on participants who are not tobacco-free. If you are unable to participate in any of the health-related activities or achieve any of the health outcomes required to earn an incentive or avoid a surcharge, you may be entitled to a reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by contacting the Office of the Plan Administrator for the Allstate Medical Plan at 2775 Sanders Rd. Suite F5, Northbrook, IL 60062.

The information from your WBA and the results from your biometric screening will be evaluated by Virgin Pulse and will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as wellbeing and health risk coaching. You also are encouraged to share your results or concerns with your own doctor. The information from your WBA and results from biometric screening will also be provided to Truven, our data warehouse vendor and may be used to provide aggregate data reporting and analytics. Virgin Pulse will also report your completion of the WBA (but no other personally identifiable information) to the Allstate Benefits Center which will, in turn, provide the information to the Company so you will not be assessed the Wellbeing Assessment surcharge.

Protections from Disclosure of Medical Information

We are required by law to maintain the privacy and security of your personally identifiable health information. Although the wellness program and Allstate may use aggregate information it collects to design a program based on identified health risks in the workplace, the Allstate Good Life Wellness Program will never disclose any of your personal information either publicly or to the employer, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information that personally identifies you that is provided in connection with the wellness program will not be provided to your supervisors or managers and may never be used to make decisions regarding your employment.

Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the wellness program, and you will not be asked or required to waive the confidentiality of your health information as a condition of participating in any component of the wellness program, receiving an incentive, or avoiding a surcharge. Anyone who receives your information for purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements. The only individuals who will receive your personally identifiable health information are Virgin Pulse, Truven, Total Wellness (for individuals who had on-site biometric screenings) and Premise Health (for individuals who obtained their biometric screening from an on-site wellness center) in order to evaluate the results and to provide you with services under the wellness program.

In addition, all medical information obtained through the wellness program will be maintained separate from your personnel records, information stored electronically will be encrypted, and no information you provide as part of the wellness program will be used in making any employment decision. Appropriate precautions will be taken to avoid any data breach, and in the event a data breach occurs involving information you provide in connection with the wellness program, you will be notified as soon as possible.

You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate.
