Health Risk Coaching Program
Dealing with a chronic health condition — like diabetes or chronic pain — can often be physically, emotionally and financially challenging.
That’s why the Allstate Medical Savings Plan and Allstate Medical Value Plan coverage options offer health risk coaching programs. These programs are designed to help those with chronic conditions deal with their day-to-day health concerns in ways that won’t take much time or money.
Should you or an enrolled dependent have or develop a chronic condition that would benefit from health risk coaching, you may be contacted by the Allstate Medical Plan’s third-party administrator or a program vendor (listed below) about their services:
- Aetna: In Touch Care program; or
- Blue Cross Blue Shield of Illinois: Blue Care Connection® Condition Management and Wellness program
- Livongo for diabetes and hypertension-high blood pressure (NEW)
- Accordant Care (a CVS company) for 19 chronic conditions (NEW)
These programs can help support you in managing your chronic conditions by providing coaching and information on topics relevant to the condition such as medications, exercise or diet — they can also help navigate the health care system as well help you in making informed health care decisions.
Coaching is personalized, and is designed to help you achieve a better overall health care experience, while also potentially saving you, the Medical Plan and the Company money. There is no additional charge to those who participate and eligibility is based on claims information which includes hospital, provider and/or prescription billing/claims data— not on personal medical records.
You are eligible for the health risk coaching program only if identified by your third-party administrator.
If you are contacted about the program but would prefer to opt-out, just answer the phone and let your third-party administrator know that you do not want to participate.
All interactions you have while participating in the health risk coaching program are completely confidential. In accordance with Health Insurance Portability and Accountability Act of 1996 (HIPAA) rules, your personal health information is held in strict confidence between you and your health risk coaching support team.
