The state in which you live determines your third-party administrator for the Allstate Medical Savings Plan and Allstate Medical Value Plan options.

If You Move and Change Third-Party Plan Administrators in the Middle of the Year

Contact Advocacy Services to ensure that your deductibles and out-of-pocket maximums for the year are transferred to your new third-party administrator. They can help facilitate the transfer of these amounts from the old third-party administrator to the new one. Representatives are available at (888) 255-7772 from 7 a.m. to 6 p.m. Central time, Monday through Friday.
