Do you need life and/or accidental death and dismemberment (AD&D) coverage? Both are offered for purchase during your enrollment period.
Life insurance can give you peace of mind knowing that your loved ones will receive money to help cover their expenses if you pass away. You can purchase life insurance of up to eight times your Qualified Annual Earnings (QAE) up to $4 million.
Only available for new hires: Evidence of insurability (EOI) is waived for coverage of up to three times your QAE (up to $500,000). If you do not enroll upon your initial eligibility and subsequently want to enroll, you will be required to provide EOI.

If you enroll in life insurance, you can elect any of the following coverages:
You can elect life insurance for your spouse/domestic partner in $10,000 increments up to $250,000. The total cannot be more than your total 2019 life insurance coverage. Evidence of insurability may be required. If you are married to another employee or associate, you cannot enroll in Spouse/Domestic Partner Life Insurance if your spouse/domestic partner is enrolled in life insurance.
You can elect life insurance coverage for your child(ren) in $5,000 increments up to $25,000. If you are married to another employee or associate, your child(ren) can only be covered under one of you.
This benefit helps protect you and your loved ones from the financial setbacks that can result from accidental death or dismemberment. Coverage is available for up to eight times your QAE (maximum QAE is $125,000). No evidence of insurability is required.
